Photographs of patients and their relatives are on the website by permission only.The service complies with the Data Protection Act 1998 and the Caldicott Report. Neither party will disclose to any third party, except those involved directly in a patient’s clinical management or where required to do so by statute/DH directive, information acquired during the period of the Agreement which concerns the identity, personal details, clinical condition or treatment to any individual, without the individual’s informed consent. The service has a dedicated Caldicott Guardian and several ‘Information Guardians’ as per the PCT’s Information Governance Strategy and all staff work to the PCT’s policies on Data Protection, Data Quality, Confidentiality, Information Governance and Records Management policies and the Information Sharing Protocol developed with partners in care.

Data contained in the website will be used for the purpose of informing the public and staff about the Specialist Palliative Care and End of Life Care Services.

Information received from the public relating to their donations or letters received will be used for no other purpose and will not be passed on to any third parties.

All service records are stored securely in line with Data Protection guidance and information governance policies.
